Agencies that want to apply for the Tobacco Cessation and Prevention Initiative Community Grant Cycle 1 can now do it online.
To apply online for Community Grant funding review the
Request for Applications document, then click here to
begin the application process. You will then be guided through setting up an account with Missouri Foundation for Health. After setting up an MFH account, you will be automatically taken to the online application.
Applications may be created in more than one session, so you can do it at your own pace. When exiting the application, be sure to choose
“Save & Finish Later” to save your work. Answers can be written in other formats and then pasted into the online application. Upon completion, applicants must choose the “submit” button or the online application will not be received by MFH. In addition, applicants must submit online all required attachments listed in the RFA Application Checklist (page 10-11). These can be submitted in Word, Excel, or Adobe PDF formats.
If you have any questions regarding the online application process, please contact Matthew Kuhlenbeck at 314.345.5541 (toll-free 800.655.5560) or email mkuhlenbeck@mffh.org.
Online applications are preferred, but paper applications will also be accepted.
If you do not have the ability to upload the required attachments listed in the RFA Application Checklist, a paper copy may be submitted to:
